Service Requests

This service request form is provided for Authentic Custom Homes, LLC. clients.

Any new items that you wish to request service for under the terms of your warranty should be reported in writing to Authentic Custom Homes, LLC. using the “Service Request Form.” Please complete the form below to request service for all repairs, with the exception of those repairs that require emergency service. By submitting this form you will be helping us to expedite your needs in a timelier manner. Therefore, we ask that this form be used instead of a phone call or verbal request in all cases. All work will be initiated upon receipt of the form in our office. This allows our service department to work more accurately and efficiently. For emergency repairs please use the list of trade partners provided to you upon closing. Before submitting a Service Request to our office, please review the Warranty Section of your contract.
